Root-cause Failure Analysis

Dynaflow Research Group has performed a root-cause analysis for a cooling water system that had 3 recent leakages. The failures were in 10” pipe and 18” pipe made of fiberglass material.

Dynaflow Research Group reviewed the material data, engineering analyses and failure info and determined the root-causes of the failures. For one location the failure was attributed to poor quality of the joint, while at the other location it was found that earlier modification works at this location had increased local pipe stresses, leading to the failure.

These findings helped the client to prevent similar failures to occur in the future.
